Form 145, or "Communication of Data to the Payer," is an important document that employees must complete so that their company or employer can correctly calculate the percentage of income tax withholding applied to their paycheck.
In addition to completing it at the start of employment, it is advisable to review and resubmit it at the beginning of each year, in case there have been any changes in the employee's personal or family circumstances.
